Abstract

|
연구목적: 본 연구의 목적은 환자들의 병원 진료에서의 참여 경험을 탐색하고, 이러한 참여에 영향을 미치는 요소가 무엇인지를 이해하는 것이다.
연구방법: 본 연구는 서술적 질적 연구로서, 목적적 표집법에 의해 서울시에 소재한 3차 대학병원의 내, 외과계 병동이나 외래 부서에서 진료를 받은 21명의 환자를 대상으로 하였다. 개별적 면담을 통해 자료를 수집하였으며, 수집된 면담 자료는 탐구 현상에 대한 이해와 지식을 얻기 위해 전통적 질적 내용 분석을 실시하였다.
연구결과: 병원에서 환자들이 진료를 받으면서 참여한 경험으로 도출된 2개의 범주는 ‘반응적 참여 행위’와 ‘능동적 참여 행위’이었다. 주요 참여 행위는 ‘의료진의 지시사항에 따름’, ‘의료진에게 경청함’, ‘질문에 답하고 치료 절차에 응함’, ‘질문하기’로 나타났다. 의료진의 세심하고 친절한 태도와 설명, 의료 장비의 이용 가능성이 환자 참여를 촉진하는 것으로 나타났으며, 의료진과의 제한된 접근성, 미흡한 의사소통, 불편한 병실 환경, 환자의 다루기 힘든 통증과 참여 인식 부족은 환자 참여를 저해하는 요소이었다.
결론: 본 연구의 결과, 환자들은 병원 진료에 주로 반응적으로 참여하고 있는 것으로 나타났다. 또한 환자들의 참여를 촉진하기 위해서는 의료진의 역할이 중요하며, 이와 함께 의료 환경과 환자 요소가 고려되어야 할 것이다.

Purpose: The aim of this study was to explore patients’ experience of participation in their hospital care.
Methods: Face-to-face interviews using a semi-structured interview guide were conducted with a total of 21 patients in a tertiary hospital in Seoul, South Korea. Collected data were analyzed using a qualitative content analysis.
Results: Two categories of patients’ experience of participation in hospital care emerged: 1) Reactive participatory activity 2) active participatory activity. Major participatory activities included ‘complying with medical instructions’, ‘listening’, ‘responding’ and ‘questioning’. Healthcare professionals’ attentive attitudes and explanation, and availability of care equipment were facilitating factors affecting patient participation in their care, whereas limited accessibility to and poor communication with healthcare professionals, inconvenient patient room, and patient factors were barriers to their participation.
Conclusion: The findings in this study reveal that patients tend to participate in their care in a reactive way and that healthcare professionals have a significant role in facilitating patients’ participation. Furthermore, the care environment and patient factors need to be considered to promote patient participation in hospital care.
